The Johns Hopkins ACG System is a powerful tool that supports you in understanding and managing the unique health needs of the people you care for. Instead of relying only on medical records, it brings together data from multiple sources—giving you a more complete view of each person’s health journey

By identifying risk and tracking patients over time, the ACG System can help you plan ahead and reduce health care costs

ACG System allows you to be proactive rather than reactive when it comes to your population’s unique health care needs

It  helps you combine a population-level perspective with patient-level behaviors and conditions.

  1. Unintentional weight loss
  2. Weakness (grip strength)

  4. Low physical activity
  5. Slow walking speed
