Well pathway

This pathway has been redesigned to support dementia co-diagnosis between Primary Care (including Frailty Teams) and memory services.

Diagnosis and management can:

• Improve quality of life for the patient and family
• Increase access to support
• Reduce symptoms and extend independence with treatment
• Reduce healthcare usage
• Increase dementia diagnosis rate
