A blank ReSPECT plan should never be given to a patient as this can lead to confusion about how to complete it and who should complete it. If a patient wishes to have a blank form to look at alongside the patient information leaflet before having a conversation, they should be given a specimen blank plan which can be found here: ReSPECT_v3-1_Form_Specimen_FINAL.pdf
